Animal Intake Form
Pet Shelter Intake Form – Resident Instructions
Purpose: To help speed check‑in and ensure your pet receives appropriate care at the Town’s emergency pet shelter.
Forms:
Before You Arrive
- Complete one intake form per pet.
- Choose how to fill it out:
- Digital: Open the PDF in Google Chrome (or any modern browser), click into the fields, and type your information.
- Handwritten: Print the PDF and write clearly in blue or black ink.
- Sign where indicated. Some sections require your signature for consent to care and emergency treatment.
- Save and keep a copy for your records.
- If typed: Save As using this format: LastName_PetName_Date.pdf (e.g., Smith_Buddy_2025‑09‑15.pdf).
- If handwritten: take a clear photo or make a photocopy.
- Bring documentation listed in the checklist below.
What to Bring (Checklist)
- Completed pet intake form (one per pet)
- Photo ID (owner/guardian)
- Vaccination records (rabies tag/certificate; other vaccines if available)
- Medications (labeled with pet’s name, dosage, schedule)
- Food (3–5 days’ supply) and feeding instructions
- Crate/carrier (required for cats, small animals; recommended for dogs)
- Leash/harness (no retractable leashes, if possible) and a collar with ID
- Comfort items (blanket, toy) and waste bags/cat litter & tray
- Vet information (clinic name, phone)
- Recent photo of your pet (on your phone is fine)
Tip: Label items with your Last Name + Pet Name.
Day‑of Shelter Check‑In
- Go to the Pet Shelter Entrance/Reception. Follow posted signs; pet shelter is separate from the human shelter.
- Present your completed form(s) and ID to staff/volunteers.
- Leash/crate your pet before exiting your vehicle.
- Receive your pet’s ID band/kennel number and review care instructions with staff.
Important Notes
- One form per pet. Attach extra pages if needed for medical/behavioral notes.
- Legibility matters. If handwriting, print clearly so staff can read critical info.
- Behavior/Special Needs: Clearly note bite history, triggers, allergies, and mobility needs.
- Medications: Provide dosing schedule; do not pre‑mix in food unless instructed.
- Service Animals: Service animals are accommodated; identify them during check‑in.
- Sick/Injured Pets: Inform staff immediately upon arrival.
- Owner Responsibility: Owners remain legally responsible for their pets unless otherwise directed by incident command.
Privacy & Use of Information
Information collected is used only for animal identification, care, reunification, and safety within the sheltering operation.
If You Can’t Print
- Visit your local library/town office to print, or complete the form on‑site (allow extra time).
