Narragansett Fire Department Service Complaint Form

  1. To ensure the integrity of its operations and personnel, the Narragansett Fire Department will investigate all complaints made in person, by phone, mail or email, to include anonymous complaints.

    Any complaint of alleged misconduct will be forwarded to the Office of Professional Standards (OPS), which falls under the responsibilities of the Administrative Fire Captain. This includes complaints originating from either outside of the Department (i.e., civilian complaints), or from within the Department (i.e., from Department members such as civilian workers; complaints filed by other firefighters; or allegations of misconduct that are initiated by supervisors).

    A complaint package, that contains a complaint procedure form describing the Department’s complaint procedures and a complaint of firefighter misconduct form, are available for the public in the following locations: (Spanish version available)

    3. The Narragansett Town Hall / Office of Town Manager
  2. Please state your complaint, including names, times, locations, witnesses and any other information that would help in investigating your complaint. If employee's names are unknown, explain what each employee looked like.

  3. Please provide specific details about the firefighter's actions or the department's response. Examples may include unprofessional behavior, lack of response, or inappropriate conduct.

  4. Please attach any relevant documents, such as photographs or written statements, to your complaint.

  5. Submission Instructions:
